Microglial TRPV1 in epilepsy: Is it druggable for new antiepileptic treatment?

چکیده

Epilepsy is one of the most common neurological diseases worldwide with a high prevalence and unknown pathogenesis. Further, its control challenging. It generally accepted that an imbalance between excitatory inhibitory properties central nervous system (CNS) leads to large number abnormally synchronized neuronal discharges in brain. Transient receptor potential vanilloid protein type 1 (TRPV1) non-selective cation channel contributes regulation influences excitability system. This includes release neurotransmitters, action generation due alterations ion channels, synaptic transmission, changes glial cells. There abundant evidence TRPV1 widely expressed (including microglia) involved development epilepsy through neuroinflammation. In conclusion, microglial participates neuroinflammatory reactions functions as proinflammatory mediator. presents novel treatment approach regulate seizures brought on by

Refractory epilepsy: treatment with new antiepileptic drugs

Five antiepileptic drugs have been marketed in the last decade. We report here a retrospective study of patients attending our unit who were prescribed one of the new antiepileptic drugs. All these patients had refractory localization related epilepsy and had failed to respond to a first-line drug. The drugs had a different profile of side-effects but topiramate (42%) was the most common drug t...

Adjunctive therapy in epilepsy with new antiepileptic drugs: is it of any value?

A total of 97 patients were recruited into a prospective, follow-up study after they were prescribed an adjunctive antiepileptic drug. The patients were followed up over a 6-month period. The interview included questions on Quality of Life, side-effects, adverse events and seizure frequency and severity. We operationally defined patients 'satisfaction' as (i) still on new drug; (ii) experiencin...

Optimizing antiepileptic drug treatment in tumoral epilepsy.

Between 30% and 50% of patients with brain tumors first present with a seizure, and up to 30% more will develop seizures later. Therefore, optimal management of these patients requires a rational approach to the use of antiseizure medications. Based on current evidence, prophylactic prescription of long-term antiepileptic drugs (AEDs) in patients with brain tumors in patients who did not presen...

Antiepileptic Drug Treatment in Children with Epilepsy

Most children with new-onset epilepsy achieve seizure freedom with appropriate antiepileptic drugs (AEDs). However, nearly 20 % will continue to have seizures despite AEDs, as either monotherapy or in combination. Despite the growing market of new molecules over the last 20 years, the proportion of drug-resistant epilepsies has not changed. In this review, we report the evidence of efficacy and...

Principles of antiepileptic drug treatment of epilepsy.

For the successful treatment of epilepsy, accurate diagnosis of epilepsy and epileptic seizures, and proper selection of antiepileptic drugs (AED) according to the classification of epileptic syndromes are fundamentally important. Efficacy of AED treatment, however, depends not only on its pharmacological action but also on its efficient use, namely a rationally thinking tailor-made treatment c...
